Enrollment Application

Welcome to the online application form for enrollment in the College of Graduate Health Sciences (CGHS) Summer Research Scholars program at the University of Tennessee Health Science Center (UTHSC). Eligibility in the SRS programs requires completion of one semester of general biology and/or chemistry, official transcripts, and 2 letters of recommendation from your college professors.

This online application will take some time to complete. You may return to it as often as you wish to enter information; however, once the application is submitted, changes can be made only by contacting the SRS administrator at 901.448.4465 or via email at shampto4@uthsc.edu.
